BACKGROUND: The Global Campaign against Headache is conducting a series of population-based studies to fill the large geographical gaps in knowledge of headache prevalence and attributable burden. One major region not until now included is South America. Here we present a study from Peru, a country of 32.4 million inhabitants located at the west coast of South America, notable for its high Andes mountains. METHODS: The study was conducted in accordance with the standardized methodology used by the Global Campaign. It was a cross-sectional survey using cluster randomised sampling in five regions to derive a nationally representative sample, visiting households unannounced, and interviewing one randomly selected adult member (aged 18-65 years) of each using the Headache-Attributed Restriction, Disability, Social Handicap and Impaired Participation (HARDSHIP) questionnaire translated into South American Spanish. The neutral screening question ("Have you had headache in the last year?") was followed by diagnostic questions based on ICHD-3 and demographic enquiry. RESULTS: The study included 2,149 participants from 2,385 eligible households (participating proportion 90.1%): 1,065 males and 1,084 females, mean age 42.0 ± 13.7 years. The observed 1-year prevalence of all headache was 64.6% [95% CI: 62.5-66.6], with age-, gender- and habitation-adjusted prevalences of 22.8% [21.0-24.6] for migraine (definite + probable), 38.9% [36.8-41.0] for tension-type headache (TTH: also definite + probable), 1.2% [0.8-1.8] for probable medication-overuse headache (pMOH) and 2.7% [2.1-3.5] for other headache on ≥ 15 days/month (H15+). One-day prevalence of headache (reported headache yesterday) was 12.1%. Migraine was almost twice as prevalent among females (28.2%) as males (16.4%; aOR = 2.1; p < 0.001), and strongly associated with living at very high altitude (aOR = 2.5 for > 3,500 versus < 350 m). CONCLUSION: The Global Campaign's first population-based study in South America found headache disorders to be common in Peru, with prevalence estimates for both migraine and TTH substantially exceeding global estimates. H15 + was also common, but with fewer than one third of cases diagnosed as pMOH. The association between migraine and altitude was confirmed, and found to be strengthened at very high altitude. This association demands further study.

Objetivo: Determinar la asociación entre funcionalidad familiar y deterioro cognitivo leve en la familia con adulto mayor. Metodología: Diseño transversal analítico en familias con pacientes geriátricos. Se consideró familia con paciente geriátrico cuando al menos uno de sus integrantes tenía más de 60años de edad. Los grupos de comparación fueron la familia con paciente geriátrico sin deterioro cognitivo y la familia con paciente geriátrico con deterioro cognitivo leve determinada con el instrumento MoCA. La funcionalidad familiar se evaluó con el instrumento APGAR familiar, el cual identifica tres categorías: funcionalidad familiar, disfuncionalidad familiar moderada y disfuncionalidad familiar severa. El análisis estadístico incluyó chi cuadrado y prueba de Mann Whitney. Resultados: En la familia con paciente geriátrico, en el grupo sin deterioro cognitivo la prevalencia de funcionalidad familiar es del 89,7% y en el grupo con deterioro cognitivo leve la prevalencia de funcionalidad familiar es del 59,3% (MW=4,87, p<0,000). Conclusión: Existe asociación entre funcionalidad familiar y deterioro cognitivo leve.(AU)

BACKGROUND: Sleep disturbances are highly prevalent in depressive episodes and are linked to higher mood severity and suicidal behaviors. Slow wave sleep (SWS) and REM sleep are compromised in depression. Current evidence suggests that rapid antidepressant effects of intravenous (IV) ketamine in patients with treatment-resistant depression (TRD) is mediated by its effects on SWS and REM sleep. Sleep phenotypes may help predict ketamine response. METHOD: In this observational study, we investigated differences in rates of response among sleep phenotypes defined by QIDS-SR in a cohort of patients with TRD (n = 52) treated with IV ketamine or intranasal (IN) esketamine. Also, we explored a neurovegetative symptoms of atypical depression (NVSAD) phenotype and its association between response and change in QIDS-SR following the treatment with IV ketamine/IN esketamine. RESULTS: 94 % of patients reported sleep difficulties and 62 % reported more than one sleep phenotype with middle and early insomnia being the most prevalent. Individuals with baseline hypersomnia showed higher response rates and more pronounced improvements on their QIDS-SR score. Additionally, 15 % of patients presented with NVSAD phenotype; the majority of whom achieved response and had higher reductions on QIDS-SR. A trend towards faster response was identified for hypersomnia and atypical depression phenotypes. LIMITATIONS: Observational study design and lack of a placebo group. CONCLUSIONS: Our data indicate that patients with TRD who have baseline hypersomnia and atypical depression features experienced a more substantial reduction in depressive symptoms and are more likely to achieve response with ketamine/esketamine. This could serve as a future predictor for clinical response.

The objectives of this study were: 1) to estimate the prevalence of child abuse (CA), intimate partner violence (IPV), and elder abuse (EA) in a representative sample of older Mexican women by status, type, and cause of disability; and 2) to examine the associations of EA with CA, IPV, and disability status. We conducted a secondary data analysis of 21,718 women aged 60 years and older. Older women with disabilities had higher prevalences of CA, IPV, and EA than those with some difficulties or without disabilities. In older women whose disability was acquired at birth or due to aggression, this prevalence was especially high. Older women with disabilities and a history of CA and IPV were twice as likely to suffer EA. Policymakers and government leaders should incorporate the matter of violence against women with disabilities into their public agendas. This should be done while considering the women's various disabilities and causes of disability.

BACKGROUND: Mineral intake may protect against cognitive impairment (CI) and all-cause dementia, which affects a large number of adults worldwide. The aim of this study was to investigate the association between mineral intake and Montreal Cognitive Assessment (MoCA), which is a sensitive and specific test. METHODS: In total, 201 adults were included in a cross-sectional study. They completed a three-day dietary record to estimate their average daily intake of minerals. Contributions to dietary reference intakes (DRIs) were also calculated. The participants were divided into tertiles according to their mineral intake. CI classifications were determined via the MoCA (score < 26). Apolipoprotein E (APOE) genotyping was carried out, and the patients' anthropometric measurements and physical activity, health and personal data were collected. RESULTS: The prevalence of CI in this selective sample was 54.2% (34.3% females and 19.9% males). In women, being in the third tertiles of iron and manganese intake was associated with lower odds of having CI (OR [95% CI]: 0.32 [0.11 ± 0.93]; 0.33 [0.12 ± 0.93], p < 0.05). No significant differences were observed for any of the nutrients studied in men. CONCLUSIONS: These findings suggest that a low mineral intake, especially low iron and manganese intake in women, is associated with a worse cognition as assessed by MoCA.

Introduction: Young adults move from adolescence into adulthood, and they face physical, social and personal changes that can affect their health. Regarding their nutritional needs, their energy requirements are lower than during adolescence, but they may need the same or higher amounts of other essential nutrients, such as folate, vitamin C, vitamin D, calcium, and iron. Furthermore, their increasingly sedentary lifestyle has led to a high prevalence of overweight and obesity in this group. However, despite being a vulnerable stage to nutritional problems, not enough attention has been paid to it from this point of view. In this context, eggs stand out as a food of great nutritional value for young adults, as they are an excellent source of protein, vitamins, and minerals. In addition, eggs are a nutritionally dense food, which makes them particularly useful in weight control and in situations with low energy requirements but high demands for other nutrients. Moderate egg consumption as part of a balanced diet helps to follow a healthy diet in this population group, contributing to improve their current and future health.


Introducción: Los adultos jóvenes, en su paso desde la adolescencia hacia la vida adulta, deben afrontar cambios físicos, sociales y personales que pueden afectar a su salud. Sus requerimientos energéticos son menores que durante la adolescencia, pero pueden necesitar la misma cantidad o mayor de otros nutrientes esenciales, como folatos, vitamina C, vitamina D, calcio y hierro. Además, su estilo de vida cada vez más sedentario ha llevado a una alta prevalencia de sobrepeso y obesidad en este grupo. Sin embargo, a pesar de ser una etapa sensible a los problemas nutricionales, no se le ha prestado suficiente atención desde este punto de vista. En este contexto, el huevo destaca como un alimento de gran valor nutricional para los adultos jóvenes, dado que es una excelente fuente de proteínas, vitaminas y minerales. Además, el huevo es un alimento de alta densidad nutricional, lo que lo hace especialmente útil en el control de peso y en situaciones de bajas necesidades energéticas pero altas con respecto a otros nutrientes. El consumo de huevo en cantidades moderadas y como parte de una dieta equilibrada ayuda a seguir una dieta saludable en este grupo de población, lo que contribuye a mejorar su salud actual y futura.

Introduction: Introduction: in the last decades, a significant increase in the incidence of Alzheimer's disease (AD) has been observed. Currently, there are no effective pharmacological treatments available, which makes preventive measures particularly important. Objective: to analyze the available scientific evidence on nutritional and dietary factors and their association with AD prevention. Methods: a review of nutritional and dietary factors related to the prevention of AD was conducted. Results: several dietary components have been positively associated with AD prevention, including intake of complex carbohydrates, fiber, omega-3, plant-based proteins, vitamins (folates, choline, vitamin D, C, B6 and B9), zinc, some bioactive compounds such as flavonols and probiotics. Following the Mediterranean, Dietary Approaches to Stop Hypertension (DASH) y Mediterranean-DASH Intervention for Neurodegenerative Delay (MIND) diets has been associated with a lower risk of developing the disease. Conclusion: nutrition may have a protective role against AD, although further studies are needed in this regard.


Introducción: Introducción: en las últimas décadas, se ha observado un incremento significativo en la incidencia de la enfermedad de Alzheimer (EA). En la actualidad, no se dispone de tratamientos farmacológicos efectivos, lo que hace que las medidas preventivas sean particularmente importantes. Objetivo: analizar la evidencia científica disponible sobre factores nutricionales y dietéticos y su asociación con la prevención de la EA. Métodos: se llevó a cabo una revisión de la literatura científica sobre factores nutricionales y dietéticos relacionados con la prevención de la EA. Resultados: diversos componentes de la dieta se han asociado positivamente con la prevención de la EA. Estos incluyen hidratos de carbono complejos, fibra, omega-3, proteínas de origen vegetal, vitaminas (folatos, colina, vitamina D, C, B6 y B9), zinc y algunos compuestos bioactivos como los flavonoles, y los probióticos. El seguimiento de las dietas mediterránea, Dietary Approaches to Stop Hypertension (DASH) y Mediterranean-DASH Intervention for Neurodegenerative Delay (MIND) se ha asociado con un menor de riesgo de desarrollar la enfermedad. Conclusión: la nutrición podría tener un papel protector frente a la EA, aunque se requieren más estudios al respecto.

Introduction: Women deserve special attention due to the different requirements they have in comparison with men. In the same way, transgender women need individualized attention. Transgender women are at greater risk of suffering heart attacks or ischemic accidents, among other diseases, and are also at greater risk of developing eating disorders, poorer weight control and poorer perception of their image. Hormone therapy for gender reaffirmation in trans women modifies their body composition, and may also increase the risk of suffering from some pathologies. The nutritional needs of cisgender women are different from those of men, and trans women also have special needs, which may depend on whether or not they follow hormone therapy. Dietary studies, although scarce, suggest that transgender women have poor dietary habits and lifestyle. It is necessary to deepen the study of the nutritional situation of the female group, considering its diversity, and to develop standards and references appropriate to each case that allow better attention to the needs of these groups.


Introducción: El colectivo femenino merece una especial atención debido a los diferentes requerimientos que tienen en comparación con el colectivo masculino. De la misma manera, se necesita atender de manera individualizada al colectivo transgénero. En concreto, las mujeres trans tienen más riesgo de sufrir infartos o accidentes isquémicos, entre otras enfermedades, y también tienen mayor riesgo de desarrollar trastornos del comportamiento alimentario y peor control de peso y percepción de su imagen. La terapia hormonal de reafirmación de género en mujeres trans modifica su composición corporal y también puede aumentar el riesgo de padecer algunas patologías. Las necesidades nutricionales de las mujeres cisgénero son diferentes a las de los varones, y también tienen necesidades especiales las mujeres trans, que pueden depender del seguimiento o no de un tratamiento hormonal. Los estudios dietéticos, aunque escasos, sugieren que las mujeres transgénero tienen unos hábitos dietéticos y un estilo de vida menos saludables. Es necesario profundizar en el estudio de la situación nutricional del colectivo femenino, atendiendo a la diversidad del mismo, y desarrollar estándares y referencias adecuadas a cada caso que permitan una mejor atención de las necesidades de estos colectivos.

Introduction: Introduction: dysphagia is classified under "symptoms and signs involving the digestive system and abdomen" in the International Classification of Diseases (ICD-10, code R13). It is defined as obstruction in the region of the pharynx, sternum or xiphoid because of obstruction of the passage of food from the mouth to the stomach or cardia, and has become one of the most important causes affecting the adequate intake of energy and nutrients. An optimal dietary and nutritional approach plays a crucial role in the management of dysphagia. The main goal of such an approach is to ensure safe and sufficient nutrition to prevent malnutrition and complications associated with dysphagia. Objectives: to establish an action protocol for an effective dietary and nutritional approach in patients with dysphagia based on the existing scientific evidence. Methods: a review of the scientific literature on the dietary-nutritional approach to dysphagia was carried out. Results: the dietary and nutritional approach in patients with dysphagia requires a multidisciplinary and personalized approach and is essential to improve the quality of life of patients with dysphagia. Through the implementation of an evidence-based action protocol, it is possible to guarantee preventing aspiration and ensuring safe swallowing and modification of textures of different foods (thus promoting adequate intake of nutrients and fluids to avoid malnutrition and dehydration). Conclusion: adequate and protocolized management, from a dietary and nutritional point of view, can have a significant impact on the quality of life of patients, improving their well-being and preventing complications associated with this condition.


Introducción: Introducción: la disfagia se clasifica dentro de los "síntomas y signos que involucran el sistema digestivo y el abdomen" en la Clasificación Internacional de Enfermedades (CIE-10, código R13). Dicho síntoma produce en la región de la faringe, el esternón o el xifoides como resultado de la obstrucción del paso de alimentos de la boca al estómago o al cardias, y se ha convertido en una de las causas más importantes que afecta a la ingesta adecuada de energía y nutrientes. Un abordaje dietético y nutricional óptimo desempeña un papel crucial en el manejo de la disfagia. El objetivo principal de dicho abordaje es garantizar una alimentación segura y suficiente para prevenir la desnutrición y las complicaciones asociadas a la disfagia. Objetivos: establecer un protocolo de actuación para realizar, de forma eficaz, el abordaje dietético y nutricional en pacientes con disfagia en base a la evidencia científica existente. Métodos: se llevó a cabo una revisión de la literatura científica sobre el abordaje dietético-nutricional de la disfagia. Resultados: el abordaje dietético y nutricional en pacientes con disfagia requiere un enfoque multidisciplinar y personalizado, y es fundamental para mejorar la calidad de vida de los pacientes con disfagia. A través de la implementación del protocolo de actuación basado en la evidencia, se puede garantizar: prevenir la aspiración y asegurar una deglución segura y, modificación de texturas de los diferentes alimentos (promoviendo de esta forma la ingesta adecuada de nutrientes y líquidos para evitar la desnutrición y la deshidratación). Conclusión: un manejo adecuado y protocolizado, desde el punto de vista dietético y nutricional, puede llegar a tener un impacto significativo en la calidad de vida de los pacientes, mejorando su bienestar y previniendo complicaciones asociadas a esta condición.

INTRODUCTION: Life expectancy (LE) and Health-adjusted life expectancy (HALE) are summary indicators that reflect a population's general life conditions and measure inequities in health outcomes. The objective of this study was to identify the differences in LE and HALE by sex, age group, and state in Mexico from 1990 to 2019. Also, to evaluate whether the changes in HALE are related to sociodemographic indicators and indicators of access to and quality of health services. METHODS: A secondary analysis was performed based on the Global Burden of Disease, Injuries, and Risk Factors Study (GBD). Data were obtained for LE (by sex and state) and HALE (by sex, age group, and state) for the years 1990, 2010, and 2019. The correlations between HALE with the Socio-Demographic Index (SDI) and with the Healthcare Access and Quality (HAQ) Index were estimated for 1990 and 2019 (by total population and sex). RESULTS: LE and HALE had an absolute increase of 6.7% and 6.4% from 1990 to 2019, mainly among women, although they spent more years in poor health (11.8 years) than men. The patterns of LE and HALE were heterogeneous and divergent by state. In 2019, the difference in HALE (for both sex) between the states with the highest (Hidalgo) and the lowest (Chiapas) value was 4.6 years. CONCLUSIONS: Progress in LE and HALE has slowed in recent years; HALE has even had setbacks in some states. Gaps between men and women, as well as between states, are persistent. Public and population policymaking should seek to lengthen LE and focus on ensuring that such years are spent in good health and with good quality of life.

Determinar el gasto de bolsillo en salud en las familias con diabetes mellitus y/o hipertensión arterial y el porcentaje del ingreso familiar durante la pandemia del Covid-19. Estudio de gasto de bolsillo en salud que incluyó muestreo consecutivo de 268 familias de México. El ingreso trimestral familiar se definió como la suma de ingresos de cada uno de los integrantes de la familia, el gasto en salud se definió como el total de erogaciones que tuvo la familia para cubrir los diferentes servicios de salud, y porcentaje de gasto en salud se definió como la relación del gasto total trimestral y el gasto corriente del hogar, valores expresados en pesos mexicanos. El promedio trimestral del gasto de bolsillo en salud en la familia con diabetes mellitus y/o hipertensión arterial en la dimensión consulta fue $975,82 y en la dimensión medicamentos $1,371.22; el gasto promedio total trimestral fue $3,133.08. El ingreso trimestral de la familia después de la pandemia del covid-19 fue $85,348.86 lo que representa 5,93% menos del ingreso trimestral antes de la pandemia. El gasto trimestral en salud fue $3,133.08, lo cual corresponde a 3,45% y 3,67% del ingreso trimestral familiar antes y después de la pandemia del Covid-19 respectivamente (AU)


Determine out-of-pocket health spending in families with diabetes mellitus and/or high blood pressure and the percentage of family income during the Covid-19 pandemic. Study of out-of-pocket health spending that included consecutive sampling of 268 families in Mexico. The quarterly family income was defined as the sum of income of each of the family members, health spending was defined as the total expenses that the family had to cover the different health services, and percentage of health spending. It was defined as the relationship between total quarterly expenditure and current household expenditure, values expressed in Mexican pesos. The quarterly average of out-of-pocket health expenditure in the family with diabetes mellitus and/or arterial hypertension in the consultation dimension was $975.82 and in the medication dimension $1,371.22; The average total quarterly expense was $3,133.08. The family's quarterly income after the covid-19 pandemic was $85,348.86, which represents 5.93% less than the quarterly income before the pandemic. The quarterly health expenditure was $3,133.08, which corresponds to 3.45% and 3.67% of the family's quarterly income before and after the Covid-19 pandemic respectively (AU)

Introducción. El COVID-19 grave con foco neumónico se maneja en hospital, en esta población las secuelas físicas y funcionales posterior al egreso hospitalario son más frecuentes, involucran la calidad de vida y tienen repercusión en las actividades cotidianas, la autopercepción y el autocuidado.Objetivo. Identificar el tiempo transcurrido para la recuperación de la calidad de vida previa al evento COVID-19 en pacientes que requirieron hospitalización.Metodología. Diseño de cohorte antes-después en pacientes que requirieron hospitalización por cuadro de COVID-19. Se consideró expuesto a los pacientes después del evento COVID-19 y no expuesto al mismo paciente antes del evento. La calidad de vida relacionada con la salud se midió con el instrumento SF-36. El plan de análisis incluyó ecuación de regresión lineal y proyección del número de días transcurridos después de la hospitalización para recuperar la calidad de vida relacionada con la salud previa al evento COVID-19 a partir del egreso hospitalario. Resultados. La dimensión que tarda más días en recuperar la calidad de vida que poseía previa al evento COVID-19 es rol físico con 225 días, y la dimensión que menos días tarda en recuperar la calidad de vida que poseía previa el evento es vitalidad y función social, ambas con 44 días.Conclusión. El tiempo para la recuperación de la calidad de vida previo a la hospitalización es diferente en cada una de las dimensiones de la calidad de vida (AU)


Introduction. Severe COVID-19 with a pneumonic focus is managed in a hospital. In this population, the physical and functional sequelae after hospital discharge are more frequent, involve quality of life, and have an impact on daily activities, self-perception, and self-care.Aim. Identify the time elapsed for the recovery of the quality of life prior to the COVID-19 event in patients who required hospitalization.Methodology. Before-after cohort design in patients who required hospitalization due to COVID-19. Patients were considered exposed after the COVID-19 event and not exposed to the same patient before the event. Health-related quality of life was measured with the SF-36 instrument. The analysis plan included a linear regression equation and projection of the number of days elapsed after hospitalization to recover the health-related quality of life prior to the COVID-19 event from hospital discharge.Results. The dimension that takes the longest days to recover the quality of life that it had prior to the COVID-19 event is physical role with 225 days, and the dimension that takes the fewest days to recover the quality of life that it had prior to the event is vitality and social function, both with 44 days.Conclusion. The time for recovery of quality of life prior to hospitalization is different in each of the dimensions of quality of life (AU)

Objetivo: Identificar los factores asociados a la adherencia al tratamiento farmacológico en pacientes con diabetes mellitus tipo 2 sin hipertensión arterial. Materiales y métodos: Estudio transversal y analítico en pacientes con diabetes mellitus tipo 2 sin hipertensión arterial. Se integraron el grupo con adherencia al tratamiento (n = 145) y el grupo sin adherencia al tratamiento (n = 49), determinado por la escala de adherencia a la medicación de Morisky de 4 ítems (MMAS-4). Como factores asociados, se estudiaron sexo, vida en pareja, escolaridad, ocupación, edad agrupada en decenios y tiempo de evolución de la diabetes agrupada en menos de un año, de 1 a 5 años, de 6 a 10 años y más de 10 años. El plan de análisis estadístico incluyó la prueba de ji al cuadrado. Resultados: Se identificaron el sexo (p = 0,045), la vida en pareja (p = 0,045), la edad (p = 0,001) y el tiempo de evolución de la enfermedad (p = 0,001) como factores asociados a la adherencia al tratamiento farmacológico. La escolaridad no se identificó como un factor asociado a la adherencia terapéutica en el paciente con diabetes mellitus tipo 2 (p = 0,289). A mayor edad, menor adherencia al tratamiento farmacológico en el paciente con diabetes: el punto de corte se presenta a los 60 años; después de esta edad, el porcentaje en el grupo no adherente es mayor que en el grupo con adherencia (p = 0,001). A mayor tiempo de evolución de la diabetes, menor probabilidad de adherencia; en el grupo no adherente, el porcentaje de pacientes con más de 10 años de evolución es 67,3 % y en el grupo con adherencia el porcentaje corresponde a 33,8 % (p = 0,001). Conclusiones: Se identificaron los factores asociados a la adherencia al tratamiento farmacológico en pacientes con diabetes mellitus tipo 2 sin hipertensión arterial.


Objective: To identify the factors associated with adherence to drug therapy among patients with type 2 diabetes mellitus without hypertension. Materials and methods: A cross-sectional, analytical study conducted with patients with type 2 diabetes mellitus without hypertension, divided into an adherent group (n = 145) and a non-adherent group (n = 49), determined by the 4-item Morisky Medication Adherence Scale (MMAS-4). The associated factors were sex, cohabitation, schooling, occupation, age in 10-year groups and diabetes progression time grouped in less than 1 year, from 1 to 5 years, from 6 to 10 years and more than 10 years. The statistical analysis plan included the chi-square test. Results: The factors associated with adherence to drug therapy were identified as sex (p = 0.045), cohabitation (p = 0.045), age (p = 0.001) and disease progression time (p = 0.001). Schooling was not identified as a factor associated with adherence to drug therapy among patients with type 2 diabetes mellitus (p = 0.289). The older the patient with diabetes, the lower the adherence to drug therapy; the cut-off point was 60 years of age, after which the percentage in the non-adherent group was higher than in the adherent group (p = 0.001). The longer the diabetes progression time, the lower the probability of adherence; in the non-adherent group the percentage of patients with more than 10 years of disease progression was 67.3 % and in the adherent group the percentage was 33.8 % (p = 0.001). Conclusions: The factors associated with adherence to drug therapy among patients with type 2 diabetes mellitus without hypertension were identified

Successful pregnancy for optimal fetal growth requires adequate early angiogenesis and remodeling of decidual spiral arterioles during placentation. Prior to the initiation of invasion and endothelial replacement by trophoblasts, interactions between decidual stromal cells and maternal leukocytes, such as uterine natural killer cells and macrophages, play crucial roles in the processes of early maternal vascularization, such as proliferation, apoptosis, migration, differentiation, and matrix and vessel remodeling. These placental angiogenic events are highly dependent on the coordination of several mechanisms at the early maternal-fetal interface, and one of them is the expression and activity of matrix metalloproteinases (MMPs) and endothelial nitric oxide synthases (NOSs). Inadequate balances of MMPs and nitric oxide (NO) are involved in several placentopathies and pregnancy complications. Since alcohol consumption during gestation can affect fetal growth associated with abnormal placental development, recently, we showed, in a mouse model, that perigestational alcohol consumption up to organogenesis induces fetal malformations related to deficient growth and vascular morphogenesis of the placenta at term. In this review, we summarize the current knowledge of the early processes of maternal vascularization that lead to the formation of the definitive placenta and the roles of angiogenic MMP and NOS/NO mechanisms during normal and altered early gestation in mice. Then, we propose hypothetical defective decidual cellular and MMP and NOS/NO mechanisms involved in abnormal decidual vascularization induced by perigestational alcohol consumption in an experimental mouse model. This review highlights the important roles of decidual cells and their MMP and NOS balances in the physiological and pathophysiological early maternal angiogenesis-vascularization during placentation in mice.

Background: Rheumatoid arthritis affects approximately between 0.3 and 1.2% of the world population. In Latin America, different studies have estimated a prevalence between 0.2 and 0.5% in the population over 16 years of age. Objective: To identify the epidemiological profile of rheumatoid arthritis. Material and methods: Descriptive cross-sectional design carried out in an urban population of a social security institution in Mexico. The information of the clinical file of 373 patients was studied. The epidemiological profile included the sociodemographic dimension, family history, health, clinical, therapeutic, biochemical, extra-articular manifestations and complications. Statistical analysis percentages, means, confidence intervals for percentages and confidence intervals for averages were calculated. Results: The wrists were the most affected joints with 44.6% (95% CI: 39.5-49.6%). The extra-articular manifestation with the highest prevalence was asthenia with 9.9% (95% CI: 6.9-12.9%); predominant diagnosis according to ICD-10 was seropositive rheumatoid arthritis with 59.8% (95% CI: 54.8-64.8%), and the rheumatoid factor was highly positive in 78.3% (95% CI: 74.1-82.5%); predominant treatment was with combined therapy at diagnosis in 97.6% (95% CI: 96.0-99.1%). The duration of treatment was > 10 years in 34.1% (95% CI: 29.2-38.8%). Conclusion: This work has described the epidemiological profile of the patient with rheumatoid arthritis in different dimensions.


Introducción: la artritis reumatoide afecta aproximadamente entre 0.3 y 1.2% de la población mundial. En Latinoamérica diferentes estudios han estimado una prevalencia entre 0.2 y 0.5% en población mayor de 16 años de edad. Objetivo: identificar el perfil epidemiológico de la artritis reumatoide. Material y métodos: diseño transversal descriptivo llevado a cabo en población urbana de una institución de seguridad social en México. Se estudió la información del expediente clínico de 373 pacientes. El perfil epidemiológico incluyó la dimensión sociodemográfica, antecedentes heredofamiliares, de salud, clínicos, terapéuticos, bioquímicos, de manifestaciones extraarticulares y de complicaciones. Se calcularon porcentajes, promedios, e intervalos de confianza para porcentajes y promedios. Resultados: las muñecas fueron las articulaciones más afectadas con 44.6% (IC 95%: 39.5-49.6%). La manifestación extraarticular con más alta prevalencia fue la astenia con 9.9% (IC 95%: 6.9-12.9%); el diagnóstico predominante de acuerdo con el CIE-10 fue la artritis reumatoide seropositiva con 59.8% (IC 95%: 54.8-64.8%) y se encontró el factor reumatoide positivo alto en un 78.3% (IC 95%: 74.1%-82.5%); el tratamiento predominante fue con terapia combinada al diagnóstico en un 97.6%, (IC 95%: 96.0-99.1%). La duración del tratamiento fue > 10 años en el 34.1% (IC 95%: 29.2-38.8%). Conclusión: este trabajo ha descrito el perfil epidemiológico del paciente con artritis reumatoide en diferentes dimensiones.

Objetivo: Determinar la satisfacción laboral y el nivel de conocimiento sobre higiene postural como factores asocia-dos a incapacidad prolongada en lumbalgia.Métodos: Estudio transversal analítico en pacientes con incapacidad temporal de trabajo por lumbalgia, se inte-graron dos grupos, pacientes con incapacidad prolongada y pacientes con incapacidad no prolongada. El tamaño de la muestra fue 120 por grupo. La satisfacción laboral se evaluó mediante el instrumento adaptado basado en el cuestionario s21/26 y S4/82, para la higiene postural se utilizó el cuestionario sobre higiene postural de Borrás. El análisis estadístico incluyó t student, mann whitney y Chi2. Resultados: No se encontró asociación entre incapacidad prolongada y satisfacción laboral, en el grupo con in-capacidad prolongada 42.5% refieren estar bastante satisfechos y en el grupo sin incapacidad prolongada la pre-valencia es 35.0% (p=0.154). En el grupo con incapacidad prolongada el nivel de conocimiento bajo sobre higiene postural es 87.5% y en el grupo sin incapacidad prolongada el porcentaje es 68.5% (p=0.000). Conclusión: La satisfacción laboral no es factor asociado con incapacidad prolongada por lumbalgia, el nivel de conocimiento sobre higiene postural es factor asociado a incapacidad prolongada por lumbalgia (AU)


Objective: To determine job satisfaction and the level of knowledge about postural hygiene as factors associated with prolonged disability in low back pain.Methods: Analytical cross-sectional study in patients with temporary work disability due to low back pain, two groups were integrated, patients with prolonged disability and patients with non-prolonged disability. The sample size was 120 per group. Job satisfaction was evaluated using the adapted instrument based on the s21/26 and S4/82 questionnaire, for postural hygiene the Borrás postural hygiene questionnaire was used. Statistical analysis includ-ed t student, Mann Whitney and Chi2.Results: No significant association was found between prolonged disability and job satisfaction. In the group with prolonged disability, 42.5% reported being quite satisfied, and in the group without prolonged disability, the prev-alence was 35.0% (p=0.154).In the group with prolonged disability, the low level of knowledge about postural hygiene is 87.5% and in the group without prolonged disability, the percentage is 68.5%, a statistically significant difference (p=0.000).Conclusion: Job satisfaction is not a factor associated with prolonged disability due to low back pain, the level of knowledge about postural hygiene is a factor associated with prolonged disability due to low back pain (AU)

BACKGROUND: Aging and multimorbidity (MM) are not enough to explain patient heterogeneity and outcomes. The objective of this study was to estimate the effect of multimorbidity patterns and indicators of socioeconomic, behavioral, and functional dimensions on the risk of death in a cohort of people ≥50 years old. METHODS: We analyzed a cohort of 7,342 persons ≥50 years old from the Mexican Health and Aging Study (MHAS), stratified by age groups (50-64, 65-84, ≥85 years old). MM was defined as the co-occurrence of two or more chronic diseases (CDs), and additional analysis included functional, socioeconomic, and behavioral indicators. Prevalence was estimated using descriptive analysis. Latent class analysis (LCA) was used to identify MM patterns, and logistic regression models were performed to estimate the risk of death at two and 18 years of follow-up. RESULTS: The most prevalent conditions were chronic pain, depression, and hypertension, with 60% of the subjects exhibiting MM at the initial evaluation. In all three age groups, indicators of the functional dimension were identified as risk factors for death. Economic precariousness was an additional risk factor in the 65-84 age group while living without a partner was an added risk factor in the ≥85 age group. For the 50-64 age group, "poor" self-perception of health and lack of physical exercise were identified as long-term risk factors for death. CONCLUSION: MM is a complex phenomenon that requires the implementation of age-specific care models. Health, socioeconomic and behavioral conditions should be considered to mitigate the risk of premature death.
